New Analysis: 42% of PHI (Protected Health Information) leaks come from manual redaction mistakes

A new analysis from i-GENTIC AI reveals that redaction mistakes account for 42% of Protected Health Information (PHI) leaks across healthcare systems.

The case study focused on a multi-state hospital network with more than 25 facilities that processes over 50,000 claims per month. i-GENTIC’s team analyzed 237,418 medical files, equivalent to 1.42 million pages of patient records, and identified a critical compliance bottleneck: manual redaction.

Twelve compliance specialists were spending more than 120 hours per month redacting documents by hand, leading to inconsistent accuracy and rising operational costs.

The hospital network engaged i-GENTIC AI’s services. The agentic AI for compliance firm deployed GENIE™, the first no-code, chat-to-governance operating system. In this case study, GENIE™ assigned AI agents to detect and automatically mask all 18 HIPAA identifiers.

The results were immediate:

    • 99.8% accuracy in identifying and redacting PHI
    • 93% reduction in manual processing time
    • $185,000 in annual savings
    • 4 full-time staff redeployed from repetitive tasks to higher-value work
    • The findings mirror a broader industry challenge. Healthcare data breaches cost an average of $7.42 million. Almost all involve PHI, either through hacking or IT incidents. As hospitals scale claims processing and digitize patient records, agentic AI-driven governance is becoming the difference between compliance and costly exposure.

“Compliance has always struggled with measurement – success often just means nothing bad happened. But with agentic AI, we can finally quantify value,”

“When you look at compliance through the lenses of risk, efficiency, and trust, it becomes measurable. How many violations were prevented? How much manual review was eliminated? How fast can regulators or clients sign off because they see transparent, real-time evidence of enforcement?”

“Companies that track across these dimensions don’t just save costs, they build reputational capital. They go to market faster, win contracts faster, and prove reliability faster than competitors. That’s a measurable advantage.”
